Courtney Michelle Stack, 22, turned herself into the St. Johns County Jail in connection with a traffic fatality last July, authorities said.
Stack, of Jacksonville, was charged with DUI causing a death of a human being and DUI. She was released after posting $51,000 bond.
On July 8, 2015, Henry Smith, 54, was walking northbound on U.S. 1 just south of Lewis Speedway. Stack, driving a 2013 Honda Civic, struck the victim, and authorities said Stack drove completely off the roadway before she struck Smith.
Smith died at Flagler Hospital.
Authorities said bloodwork obtained from the driver that day indicated there was cocaine and methadone in Stack’s system.

Authorities said Courtney Stack had cocaine in her system when fatal crash happened just north of St. Augustine last summer.
