11:05 P.M. UPDATE: FHP has released an incident report for the crash on Blanding Blvd at College Drive that occurred Sunday afternoon that sent 6 people including an infant to local hospitals.
According to FHP, Randy Teal of Jacksonville, 30, was driving a 2014 Jeep when he crashed into a 2016 Kia Soul where Nannette Green, 49, was the driver and Abigail Eberwein, 19, and Eberwein's infant daughter were passengers. The infant is listed in critical condition.
The Soul was rotated by the impact, striking a 2014 Toyota Camry in the center lane and a 2015 Toyota Camry in the left lane. The occupants of the 2014 Camry, Amy Shipley, 48, and Olivia Shipley, 15, did not suffer any injuries according to FHP's report. The occupants of the 2015 Camry, Mckenna Baker, 20, and Kelly Richardson, 39, suffered minor injuries according to FHP's report.
The Toyota Camry in the left turn lane then struck a 2013 Chevrolet Sonic that was also in the left turn lane. The driver of the Sonic, Kristen Hinson, 20, did not suffer any injuries according to FHP's report.
FHP is investigating if alcohol and/or distracted driving were a factor in the crash.
ORIGINAL STORY:
The Clay County Sheriff's Office was at the scene of a crash that closed both the northbound and southbound lanes of Blanding Boulevard at the intersection of College Drive Sunday afternoon.
CCSO told Action News Jax that additional Florida Highway Patrol units were called to the scene.
FHP says that there were five vehicles involved in the crash and six patients were transported to hospitals.
One of those patients was an infant in life-threatening condition, according to FHP.
FHP says that the crash occurred just before 5 p.m. when several vehicles were stopped at the intersection when a black Jeep SUV was unable to stop and caused a chain reaction rear end collision.
FHP is investigating if the driver of the Jeep was under the influence or was driving while distracted.
The driver of the Jeep suffered minor to moderate injuries, the grandmother and the mother of the infant in life-threatening condition sustained minor injuries and were in the white Kia Soul; two other people in a gray Toyota sedan also sustained minor injuries.
FHP said that the infant was transported to UF Health, while the grandmother and mother were transported and released from Orange Park Medical Center, and are in route to UF Health.
All lanes of Blanding Blvd. are open as of 8:42 p.m.
