COVID-19 Vaccine: Did you get your first dose at Clanzel T. Brown Center? Here’s how to get the second one

JACKSONVILLE, Fla. — FEB. 24 UPDATE: If you received the first dose of the COVID-19 vaccine at the Clanzel T. Brown Senior Center, you may return to the site for their second shot this Friday and Saturday.

Those who were given their first shot on Friday, Feb. 5, should return Friday, Feb. 26. If you received it on Saturday, Feb. 6, you should return Saturday, Feb. 27.

The city says you should arrive within the hour associated with the first letter of your last name during these time slots:

A - D

9 - 10 a.m.

E - H

10 -11 a.m.

I - L

11 a.m. - 12 p.m.

M - P

12 - 1 p.m.

Q - T

1 - 2 p.m.

U - W

2 - 3 p.m.

X - Z

3 - 4 p.m.

Those who are not able to visit at their assigned time may go to the site from 4 - 5 p.m.

You must bring a photo ID to verify your date of birth and the vaccination record card that was provided at their first visit. While copies of the COVID-19 Vaccination and Consent Form will be available at the site, individuals are encouraged to bring a completed form to reduce their wait time; the form can be accessed at COJ.net/CovidVaccineForm.

The City of Jacksonville says you will get reminder phone calls the day before and day of your appointment.

Mayor Lenny Curry announced that coronavirus vaccines will be made available to underserved communities in Northwest Jacksonville.

The City of Jacksonville said for two days only, the Clanzel T. Brown Community Center located at 4575 Moncrief Road will be used as a COVID-19 vaccination site.

City leaders said they received enough Pfizer vaccines for 975 first and second doses from the Florida Department of Health to vaccinate those 65 and older, first responders, and medical personnel.

The site will be open on Friday, Feb. 5 and Saturday, Feb. 6 from 9 a.m. to 5 p.m.

Mayor Curry said those born between January and June can be vaccinated on Friday, Feb. 5. There will be 500 first doses available.

Those who were born between July and December will receive the vaccine on Saturday, Feb. 6. The city said there will be 475 doses available.

  • You are asked to not come before 8 a.m. or after 4 p.m.
  • A mask must be worn at all times.
  • Only caretakers may accompany patients who require assistance to the vaccination site.
  • Patients must park in the designated parking areas; patients cannot be dropped off at the line.
  • Parking and waiting in line overnight is not permitted.

You will need to bring a photo ID and a completed consent form. The form will also be available to fill out at the site if you’re unable to complete it ahead of time.

For those who need a ride to the site, the Jacksonville Transportation Authority said it has direct service to the site via Route 3 – Moncrief. JTA says it is offering free rides to those 65 and older who are getting the vaccine. For JTA’s ReadiRide reservations: Customers should call at least 24 hours in advance of their appointment by calling 904-679-4555. Seniors will need to show their I.D. to the driver to ride free (available for both bus and ReadiRide services). The AME Ministers Alliance is also offering free rides all you have to do is call 904-739-7500.

After receiving the vaccine, patients must stay on site for at least 15 minutes to be monitored for potential reactions by healthcare and emergency professionals. While allergic reactions to the COVID-19 vaccine are rare, this safety measure is in place out of an abundance of caution.

Patients must return to Clanzel T. Brown Senior Center 21 days after the first shot to receive the second dose. Reminders about the follow-up appointment will be provided via phone call and text message.

Because of the vaccination operation, Agape Health, a City-contracted COVID-19 testing provider that normally operates out of the senior center, will temporarily move testing to the following locations:
