Friends and family are waiting for answers in the death of a Jacksonville Navy chief and mom of three.

A family friend says Andrea Washington was just pinned as a Chief Petty Officer last week.

A police report obtained by Action News Jax Tuesday reveals little information about what led to her death. The report says Jacksonville Sheriff's Office got to a home on Itani Way for a death investigation at 12:13 a.m. Monday.

A woman inside the home, identified by friends and family as Washington, was pronounced dead at 12:30 a.m.

The report lists several witnesses or complainants, but their information is heavily redacted, as is much of the report. Action News Jax searched court records and found Washington petitioned for a domestic violence injunction against a man earlier this month, saying she was kicked and that he pulled a gun on her.

A temporary order was granted Sept. 5, but the next court hearing -- set for the day her body was found -- never happened.

Action News Jax asked Jacksonville Sheriff’s Office if the man in the injunction is a person of interest in the case. A spokesperson said the investigation is active and no new information was available.
