JACKSONVILLE, Fla. — North Florida OB-GYN has notified its patients of a data breach that the health care system became aware of on July 27.
A portion of the health care provider's computer systems were affected by the cyber incident on or before April 29, according to North Florida OB-GYN.
North Florida OB-GYN promptly shut down its networked computer systems, initiated its incident response and recovery procedures, notified the Federal Bureau of Investigation and began an investigation, according to the notification sent to patients.

North Florida OB-GYN advises patients to remain vigilant by regularly reviewing their account statements, monitoring free credit reports and reporting any suspicious activity to their financial institutions activity.
Patients and employees may obtain a free copy of their credit report online at www.annualcreditreport.com, by calling toll-free 1-877-322-8228 or by mailing an Annual Credit Report Request Form to:
