BRADFORD COUNTY, Fla. — The Florida Department of Health in Bradford County and Union County, in partnership with WellFlorida Council, is asking the community to take part in the Community Health Assessment survey.
The DOH said the results will be included in the Bradford County Community Health Needs Assessment, which will “identify and prioritize the most pressing health issues” in Bradford and Union counties.

To be eligible, you must be at least 18 years old and reside or work or attend school in Bradford County or Union County.
